Corporation:
- Complete Corporate Resolution
- Complete Certification of Beneficial Owner(s)
- Copies of Articles of Incorporation
- Signature Card and Gambling Form
- If the corporation is operating under a “fictitious name,” you must supply a copy of the Business Tax Receipt issued by the city or state (except Churches)
- The corporation must be verifiable through the FL Secretary of State web site (www.sunbiz.org)
Partnership:
- Complete Partnership Resolution
- Complete Certification of Beneficial Owner(s)
- Copy of Business Tax Receipt – occupational license
- Fictitious Name Affidavit – may be obtained by advertising that you are doing business under the Florida Fictitious Name Status (not required if titling the account with the person’s Full Name)
- Signature Card and Gambling Form
Sole Proprietorship & DBAs:
- Complete Affidavit of Sole Ownership Form
- Copy of Business Tax Receipt – occupational license
- Fictitious Name Affidavit – may be obtained by advertising that you are doing business under the Florida Fictitious Name Status (Not required if titling the account with the person’s full name)
- Signature Card and Gambling Form
Clubs or Organizations:
- Complete Certificate of Authority
- Copy of minutes stating who the signers are, how many signers are required for signing checks and what type of account you want to open (checking or savings)
- Signature Card and Gambling Form
Limited Liability Corporation (LLC):
- Complete LLC Resolution
- Complete Certification of Beneficial Owner(s)
- Copies of Articles of LLC
- Signature Card and Gambling Form
- The corporation must be verifiable through the FL Secretary of State web site (www.sunbiz.org)
All signers on business accounts, Partnership, Sole Proprietorships, DBA’s, Club or Organizations must provide the following individual information:
Name and physical address (P.O. Box for mailing purposes only)
Phone Number
Drivers License
Social Security Number or Social Insurance Number
Mother’s Maiden Name
City and State of Birth
Email Address
The only time this information is not required is if the account is a government account (e.g. Schools, Towns, Courts).
Additional information may be needed depending on the individual circumstances.
